Think about the last time your parent visited a doctor. Did the cardiologist know what the orthopaedic had prescribed? Did anyone ask how they were sleeping, or whether they felt lonely? Was there a follow-up call after the appointment not a reminder, but a real check in?

A new programme at MGM Healthcare in Chennai is trying to change that. It is called Swayam, and it may be the most carefully designed geriatric care model in the city.

What is Swayam?  

Swayam, a Sanskrit word for self reliance, launched at MGM Healthcare, Chennai, on February 26, 2026. It is a structured, assessment driven geriatric care programme designed to bring medical, psychological, functional, and home based monitoring under one coordinated system.

What makes it different from a standard senior health check up is not any single feature, it is the architecture. Swayam connects outpatient consultations, senior friendly inpatient wards (including ICU), and hospital to home follow up into a single, continuous care pathway. When a patient leaves the building, the care does not stop.

Only Five Patients a Day And That is the Point

In most Indian hospital outpatient departments, a doctor sees 60 to 80 patients in a single session. For elderly patients with multiple conditions, complex medication lists, and symptoms that don't fit neatly into one specialty that is not enough time.

Swayam deliberately caps consultations at five patients per day.

Each visit includes a full multidisciplinary assessment completed in a single appointment covering not just physical health but psychological wellbeing, cognition, nutrition, mobility, sensory health, dental health, and even the patient's home environment. Depression screening and a loneliness assessment are built into every consultation as standard, not referred out to another department.

For a family that has spent years coordinating between four different specialists, this kind of unhurried, whole person evaluation is something genuinely new.

What the Doctors Say

Dr. Senathi Nanda Kishore, Clinical Director and Senior Consultant at MGM Healthcare, was direct about the cultural problem the programme is trying to solve:

"India is ageing rapidly, yet structured geriatric medicine remains underdeveloped. Ageing is often normalised as decline. Chronic pain, loneliness, reduced mobility, polypharmacy and sensory deterioration are accepted as natural. Swayam challenges this narrative. We are not merely a treatment clinic; we represent a proactive, preventive and restorative ecosystem for senior wellbeing. We are not managing old age; we are optimising healthy ageing."

Dr. P. Sivaraj, Senior Consultant in Internal Medicine and the clinical lead for Swayam, was equally pointed:

"Current geriatric care is often fragmented across specialties, reactive rather than preventive, with limited psychological and social assessment. Medication management is seldom well integrated, and follow-up rarely extends into structured home-based care. In contrast, we offer Chennai's most structured multidisciplinary geriatric programme, built on a true continuity of care model spanning outpatient, inpatient and home care."

The emphasis, Dr. Sivaraj explained, is on monitoring a patient's condition steadily over time, not waiting until something goes wrong before stepping in.

What Happens After the Hospital Visit?

This is where Swayam does something that few programmes in India have attempted seriously.Every quarter, a team of a doctor, nurse, and physiotherapist travels to the patient's home to conduct a structured health assessment. For patients in assisted living facilities, the same service applies.

This matters because the gap between a hospital discharge and the next appointment is where things go wrong. Medications get confused. A fall happens. Depression deepens quietly. For elderly patients without reliable transport or a family member who can take time off work, getting back to a clinic in time is genuinely difficult.

Swayam's quarterly Mobile Health Services close that gap, not with a phone call, but with a real clinical visit.

The consequences are predictable: missed diagnoses, dangerous drug interactions, untreated depression, preventable falls, and repeated emergency admissions that could have been avoided with earlier, more coordinated care.

Swayam does not claim to solve all of this. But it offers a model that takes the whole person seriously their body, their mind, their home, and their need to feel seen rather than processed.
