India vs. UK for Surgery: Is It Worth Travelling Instead of Waiting on the NHS?

Published: April 21, 2026

You've been told you need surgery. Your GP has referred you. You're on the waiting list.

Then you find out the wait is fourteen months. Or eighteen. Or, in some cases across England, more than two years.

For a knee replacement that's making it hard to walk. For a spinal procedure that leaves you waking up in pain every night. For a cardiac review that your consultant says shouldn't wait. The system, overwhelmed and under-resourced, has placed it on a list.

At some point, a question naturally arises: is there another way?

There is. Thousands of British patients travel to India each year for procedures they cannot access quickly or affordably through the NHS or UK private healthcare. They return with the surgery done, the recovery underway, and often a significant sum of money still in their account.

This post addresses that question honestly. We'll look at NHS waiting times, UK private costs, what India offers as an alternative, and exactly what HOSPIDIO manages so you never have to navigate it alone. We're not here to criticise the NHS. It is a remarkable institution. We're here to offer a practical option to patients whose health cannot wait for it.

"Your health should not have to wait eighteen months for a bureaucratic system to catch up with it."

The NHS Waiting List Crisis: What the Numbers Actually Say

The NHS waiting list crisis is not new. It has reached a scale that is genuinely alarming for patients who need elective and semi-urgent procedures.

As of 2025, NHS England's waiting list stands at over 7.5 million people. The official 18-week target for referral to treatment is being missed for a significant proportion of patients. For specific procedures, the delays are even more severe:

ProcedureTypical NHS Waitlist (2025)In India (Through HOSPIDIO)
Knee Replacement12 to 24 months 3 to 5 days
Hip Replacement12 to 20 months 3 to 5 days
Spinal Decompression12 to 18 months 1 to 2 days
Cardiac Bypass (CABG)6 to 14 months 2 to 3 days
Cataract Surgery 9 to 18 months 1 to 2 days
Hernia Repair6 to 18 months 2 to 3 days
IVF (NHS-Funded) 12 to 36 months No waiting
Hip Resurfacing 12 to 24 months 1 to 2 days

Source note: NHS England waiting time data, 2025. India timelines are indicative based on HOSPIDIO's patient coordination experience and subject to individual case assessment.

For many patients, a two-year wait for a knee replacement is not simply inconvenient. It means two years of reduced mobility, worsening muscle deterioration, increasing pain medication, and a significant decline in quality of life. The health cost of waiting is real, even when the financial cost is hidden.

"I'll Just Go Private": The UK Private Healthcare Reality

The natural response for many UK patients is to consider going private. And private healthcare in the UK is certainly available, but its costs are eye-watering for most ordinary families.

ProcedureUK Private Cost (2025)Cost in India (with HOSPIDIO)
Knee Replacement £12,000 to £18,000 £2,900 to £3,200
Hip Replacement £15,000 to £22,000 £3,700 to £4,000
Spinal Surgery (Fusion) £18,000 to £28,000 £3,850 to £4,300
Cardiac Bypass (CABG) £35,000 to £45,000 £3,000 to £3,700
IVF (Single cycle) £9,000 to £15,000 £2,600 to £3,000
Cataract (Both eyes) £6,000 to £9,000 £600 to £1,100
Hernia Repair £7,000 to £12,000 £1,850 to £2,900

Important note: India costs shown are all-inclusive treatment estimates. Flight and accommodation costs are additional, typically £600 to £1,200 return from the UK to India, plus £20 to £40 per night for accommodation near the hospital. Even including travel, the total cost is typically 60 to 75% lower than UK private.

For a knee replacement, a UK patient paying privately might spend £14,000. The same procedure in India, performed at a JCI or NABH-accredited hospital by a surgeon trained in the UK or USA with a similar implant, costs between £2,900 and £3,200. Add £800 for flights and £500 for two weeks of accommodation, and the patient still saves between £7,000 and £9,000.

For a family that has already stretched its budget, or a patient on a fixed income, that difference is transformative.

Is Indian Healthcare Really Comparable to UK Standards?

This is the question that matters most, and it deserves a direct and honest answer.India's top hospitals, the ones HOSPIDIO works with, are not simply "good for the price." They are genuinely world-class medical institutions by any international measure. Here is why:

International Accreditation

India has more Joint Commission International (JCI)-accredited hospitals than any country in Asia, and more than most countries in the world. JCI accreditation is the same international standard used to evaluate hospitals in the USA, UK, and Europe. It is rigorous, independently audited, and requires hospitals to demonstrate consistent patient safety, clinical outcomes, and governance standards.

HOSPIDIO's partner hospitals hold JCI or NABH (India's national equivalent) accreditation. We do not work with facilities that cannot demonstrate these standards.

Internationally Trained Surgeons

A significant proportion of India's senior surgeons completed their specialist training in the United Kingdom, the United States, or Australia. Many hold dual fellowships: an Indian qualification alongside a UK or US board certification. The surgeon who performs your knee replacement in Delhi may have completed a fellowship at a London teaching hospital.

Technology and Infrastructure

India's major private hospitals have invested heavily in the latest surgical technology: robotic surgery systems, advanced imaging, dedicated intensive care infrastructure, and international patient wards designed for overseas visitors. In several areas, including robotic joint replacement and complex liver surgery, India's leading centres are at the frontier of global practice.

Why Does Treatment Cost So Much Less in India?

The cost difference is not a quality difference. It reflects the difference in operating costs between two economies. Wages, real estate, insurance, and administrative overhead are a fraction of UK costs in India. A surgeon earns well by Indian standards at a salary that is significantly lower in absolute terms than their UK counterpart. A hospital bed costs less to run. These savings are passed directly to the patient, without any compromise to the standard of care.

"I was nervous about the quality. Of course I was. But the hospital felt like a five-star hotel with an operating theatre. The surgeon had trained in London. I was treated like a person, not a number." - UK patient treated for spinal stenosis in India via HOSPIDIO.

Practical Considerations for UK Patients Travelling to India

The India Medical Visa from the UK

British passport holders apply for an India Medical Visa through the Indian High Commission or Indian Visa Application Centres (IVACs) located across the UK, including in London, Birmingham, Leicester, and other cities. The application requires your passport, medical reports confirming the need for treatment, and a hospital invitation letter, which HOSPIDIO arranges on your behalf.

Alternatively, the easier way is to apply for a e-medical visa on the official Indian e-visa government website. It allows you to fill up a form with personal details, upload a copy of the passport and the medical visa invitation letter (sent by HOSPIDIO). The e-visa is issued in the form of electronic travel authorization (ETA) sent to your email. You just need to carry a copy of it with you and your passport is stamped at immigrations while entering India.

Processing time is typically 3 to 7 business days. An attendant visa is also available for one family member who wishes to accompany you.

Flights from the UK to India

Direct flights operate between London Heathrow and major Indian medical cities including Delhi, Mumbai, Chennai, and Bangalore, with carriers including Air India, British Airways, and Virgin Atlantic. Flight time is approximately 8 to 9 hours. Return economy fares typically range from £450 to £900, depending on season and how far in advance you book.

For patients travelling from Birmingham, Manchester, or other UK cities, connecting through Heathrow adds a short domestic leg. HOSPIDIO advises on the best routing based on which Indian city your hospital is in.

Recovery Time and the Return Journey

Different procedures require different lengths of stay. As a general guide:

ProcedureAverage Length of Stay
Knee/Hip Replacement 21 days total (including post-operative physiotherapy)
Spinal Surgery 15 days total
Cardiac Bypass 21 days total
IVF (Single Cycle) 25 days total (until embryo transfer)
Cataract Surgery 5 to 7 days total
Laproscopic Hernia Repair 10 to 14 days total

Your treating surgeon will confirm your specific fitness-to-fly requirements before discharge. HOSPIDIO coordinates this assessment as part of your discharge planning.

Will My UK GP Support Me?

Yes, in most cases. UK GPs are accustomed to patients who have received treatment abroad, including privately in the UK, in Europe, or further afield. When you return, your GP will need your discharge summary and operative reports, which HOSPIDIO ensures you leave India with. Your Indian specialist is also available for telemedicine follow-up, and HOSPIDIO can facilitate direct communication between your Indian team and your GP where needed.

The NHS will continue to provide your post-operative community care, physiotherapy referrals, and follow-up appointments in the UK as normal.

A note on British-Caribbean patients: If you hold a British passport and are based in Jamaica, Trinidad, Barbados, or another Caribbean country, you are equally eligible to travel to India for treatment via HOSPIDIO. The same medical visa process applies. HOSPIDIO has existing patient relationships across the Caribbean and can support your journey from your home country.

Is Travelling to India Right for You? An Honest Assessment

We will be direct: travelling to India for surgery is not the right choice for every patient. It is worth thinking through clearly before you decide. It is likely a strong option if:

  • Your procedure is elective or semi-elective: orthopaedic, cardiac, oncology, IVF, or similar
  • You are broadly fit enough to travel (your surgeon can advise)
  • Your NHS wait is 6 months or longer, and the delay is affecting your quality of life
  • UK private costs are beyond your budget
  • You have a family member who can accompany you as an attendant
  • You are comfortable communicating by WhatsApp or email to coordinate logistics

It may not be the right option if:

  • Your condition is a medical emergency requiring immediate local intervention
  • You are too medically unstable to travel by long-haul flight. Your GP can assess this.
  • Your procedure is highly unusual and requires a specialist with very specific UK-based expertise
  • You are not able to remain abroad for the required recovery period

Real Patients, Real Journeys

The best way to understand what this journey looks like is to hear from patients who have made it. HOSPIDIO has supported patients from Mauritius, Vanuatu, Kenya, the United States, and many other countries through complex treatment in India. Their stories are published in full on our blog.

"I had been waiting 16 months for a spinal decompression on the NHS. My pain was worsening and my mobility was deteriorating. After travelling to India with HOSPIDIO's support, I had the surgery within three weeks of first contact and was back home in the UK within a fortnight, with a detailed discharge report for my GP and a follow-up telemedicine call already scheduled." - Patient treated for lumbar stenosis in Delhi.

Conclusion

The NHS is a system that most British patients respect deeply, and rightly so. But a system under historic strain cannot always provide timely care for every patient who needs it. When the wait is long, the private cost is prohibitive, and your health is suffering in the meantime, travelling to India for treatment is a serious, evidence-based option, not a compromise.

India's best hospitals meet international accreditation standards. Many of their surgeons trained in the UK. The cost is a fraction of what you would pay privately in Britain. And with HOSPIDIO coordinating every step, from your medical visa to your discharge summary to your telemedicine follow-up, you are never managing this alone.

Your health does not have to wait. HOSPIDIO is here to help you move forward.
